*I recognize that Overflow Church is relying on the accuracy of the information I provide on the Team Member Work Application form. Accordingly, I assert and affirm that the information I have provided is absolutely true and correct. *I authorize Overflow Church to contact any person or entity listed on the Team Member Work Application form, and I further authorize any such person or entity to provide the Overflow Church with information, opinions, and impressions relating to my background and qualifications. *I voluntarily release the Overflow Church and any such person or entity affiliated with Overflow Church and/or listed on the Team Member Work Application form from liability involving the communication of information relating to my background or qualifications. I further authorize Overflow Church to conduct a criminal background investigation/check using a 3rd party provider of background checks if such is deemed necessary at any time. *I have read the policy and procedures of Overflow Church, and I agree to abide by them and to protect the health and safety of the children or youth assigned to my care or supervision at all times.
